Poverty plays growing role in major amputation risk
Major lower extremity amputations were once heavily concentrated in a narrow band of under-resourced, safety-net hospitals. A three-decade national study now shows that burden has spread well beyond those settings and that poverty is a more powerful predictor of limb…
Moving limb preservation forward with Shockwave Javelin Peripheral Intravascular Lithotripsy
This advertorial is sponsored by Shockwave Medical. Chronic limb-threatening ischemia (CLTI) represents the most advanced stage of peripheral artery disease (PAD) and carries a significant risk of major amputation if left untreated. Calcified tibial lesions remain among the most technically…
Deep vein arterialization challenges notion of ‘no-option’ CLTI
“Patients previously considered ‘no-option’ may no longer truly be without options,” said Anahita Dua, MD, associate professor at Harvard Medical School and a vascular surgeon at Massachusetts General Hospital. That assertion sums up the central finding of an analysis comparing…
Tibial access in claudicants linked to guideline-discordant care and CLTI progression
Retrograde tibial access has long offered technical advantages in peripheral vascular procedures, but a new study raises concern that its use in patients with claudication may be driving treatment decisions that exceed what current guidelines recommend, creating a clinically significant…
New data support tailored revascularization strategies for patients with CLTI
A new study comparing autologous vein bypass to endovascular intervention as the initial revascularization strategy for patients with chronic limb-threatening ischemia (CLTI) requiring infrapopliteal treatment found when patients were carefully matched for comorbidities, outcomes between the two approaches were largely…
Advancing below-the-knee intervention in CLTI with Shockwave Javelin Peripheral IVL
This advertorial is sponsored by Shockwave Medical. Charles Bailey, MD, George Adams, MD and Vanessa Adams, BS, share their experience with Shockwave Javelin, the first-of-its-kind forward intravascular lithotripsy (IVL) technology that is enabling further interventional treatment for patients suffering from…
Alcohol use disorder linked to higher rates of amputation and death after revascularization for CLTI
Alcohol use disorder (AUD) independently raises the risk of major adverse limb events (MALE) and mortality among patients undergoing lower-extremity revascularization for chronic limb-threatening ischemia (CLTI), according to a retrospective cohort study published in the Journal of Vascular Surgery (JVS).…
Biological sex remains a key determinant in CLTI revascularization rates and outcomes
Female patients had fewer endovascular revascularizations for chronic limb-threatening ischemia (CLTI) compared to male patients but experienced lower risks of mortality and major amputation, according to recent data. However, the findings also showed female patients were more likely to experience…
TADV delivers ‘transformative advancement’ for no-option CLTI
A systemic review and meta-analysis found transcatheter arterialization of deep veins (TADV) produces higher rates of limb salvage, amputation-free survival (AFS), and wound healing than standard-of-care (SoC) in no-option chronic limb-threatening ischemia (CLTI) patients. The findings appear in the Journal…
Study strengthens case for conservative therapy in claudication over early intervention
Early peripheral vascular intervention (PVI) for claudication is associated with higher rates of progression to chronic limb-threatening ischemia (CLTI) and major adverse limb events (MALE) compared with conservative medical management, according to data presented at the 2026 annual winter meeting of the…
